Problem

When you submit a certificate request you get the following error message:

Certificate not issued (Denied) Denied by Policy Module The DNS name is
unavailable and cannot be added to the Subject Alternate name. 0x8009480f
Certificate Request Processor: The DNS name is unavailable and cannot be
added to the Subject Alternate name. 0x8009480f (-2146875377)
Denied by Policy Module

Solution

1. Open the
   Certificate Templates console
2. Double-click your template
3. In the Subject Name tab select Supply in the request
4. Click OK
5. Submit your certificate request again
